Something in my teeth?

I hadn't drawn any portrates this year so I decided it was time to get back into practice. As always I'm not happy to let my latest artwork be less than my best and I put in a bit of effort in this one to get it just right. I've made some very specific decisions regarding layout and style that may not be seen by everybody but the end effect is undeniable.
